You are informed that the data provided to the Center will be treated in accordance with the provisions of Regulation (EU) 2016/679 of April 27, 2016, so the following treatment information is provided:

Purpose of the Treatment:

Provision of professional health services and maintenance of the clinical history of patients.


Legal basis for data processing: Law 41/2002 on patient autonomy and on rights and obligations regarding clinical information and documentation.



Data retention criteria:

The main objective of the clinical history is to facilitate health advice, recording all those data that, under medical criteria, allow accurate and up-to-date knowledge of the patient's state of health. It is an instrument intended primarily to ensure adequate patient care. Health centers have the obligation to keep clinical documentation in conditions that guarantee its correct maintenance and safety, although not necessarily in the original support, for the proper assistance to the patient for the appropriate time in each case and, at least, five years counted from the date of discharge of each healthcare process (all in accordance with article 17 of Law 41/2002, of November 14). When it is not necessary to maintain the end of the treatment, the data will be deleted with the appropriate security measures.



Transfer of data:

No data will be transferred unless a legal or judicial provision requires it. The patient agrees that when entering the melio platform and contracting the services offered by it, he consents that his data may be treated, in conditions of absolute confidentiality, by the extraction and analysis laboratories with which melio has concert, which intervene as co-responsible for data processing, and by melio's medical staff, to whom the results of the analysis will be forwarded for the preparation of the final report.



Rights that assist the patient:

Right to withdraw consent at any time.


Right of access, rectification, deletion, opposition, forgetting and portability of your data, within the limits provided by law.


Right to file a claim with the Control Authority (agpd.es) if it considers that the treatment does not comply with current regulations.



Exercise of rights:

The patient can exercise the rights of access, rectification, deletion, opposition, forgetfulness and portability, within the limits provided by Law 41/2002 at the address of the Data Controller, previously indicated.


The patient consents to the processing of their personal data in the terms set forth.
